Older adults in continuing care often live with complex, overlapping health and social needs that require holistic, person-centered approaches to assessment and care planning. This course will guide you through the process of conducting a Comprehensive Geriatric Assessment (CGA) — a structured, evidence-based approach used to identify and prioritize health concerns and inform collaborative care plans.
Through interactive modules, realistic scenarios, and practical tools, you will develop the skills to gather and interpret key information across multiple domains of an older adult’s life. By the end of this course, you’ll be equipped to contribute meaningfully to interdisciplinary care planning and advocate for the needs and preferences of older adults in your care.
